In order to provide evidence for a bug I encountered, I decided to load an auto save from when I encountered the bug. However, the game threw an error then near instantly returned to the title screen. I could not access any of my many saves before hand, even manual saves. The problem went away after I closed the game and answered the question it opened on the browser. Likely an accidental side effect of the questionnaire?

I've had the same issue.

I had loaded a few games already (say 5-6 times).

Then at one point, when I tried to load again, I had the same error as OP and the game went back to main menu.

I tried to exit the game but could not, it stayed in task bar with dark sreen. I had to end the task with the task manager.

When I launched the game back, I could open my save without any error.

I don't think it's related with the form you get when you close the game though.

I have also had the same issue - twice.
I'd describe the problem the same way as Golden - I play the game, reload from inside-session couple of times at different points (to remove visual bugs mostly, but sometimes I reloaded to fight battles differently) and suddenly every single save kicks me to to the title screen without an error message. Both times this issue has happened I wasn't even that far into the game. The second time it happened was on turn 40 (attaching the save and diagnostics (secondcase.7z). The first time it could've been... turn 80? No idea. The only save is from Turn 9 sadly, I'll attach it anyway. I might have the diagnostics, I'll attach them (firstcase.7z).
Today I tried loading one of the saves and it worked, so I suppose rebooting the PC worked (rebooting the game did not.)
I also have a black screen after exiting the game while at the same time having the questionnaire pop out in my browser. Completing it doesn't do away with the blackscreened game, I had to force-exit it every time by closing the window and making Windows force-close it.
