Finished all phases and door didn't open. Playing on Hard Mode with just Shroom and Zed. Red Dead Eye health bars were empty and displayed at bottom of screen and she still taunted. Nothing helped. Ended up offing the team to get normal end of run. I primarily use controller.

Step by step of the bug
Step 1
Had Comrade, Sweeper and Fassie (In that order) and went through headquarters to the security vault.
Step 2
Finish all 4 crystal slots going from the middle, bottom, top, then middle again.
Step 3
After the final middle slot is completed the boss is soft locked and will not die

I had finished the 4 phases of the boss fight and chipped away all of the 4 slots needed to "defeat" the boss but was locked out of the elevator as she was still alive despite having no more active slots or health. This was done in a solo playthrough with sweeper, comrade and fassie. This had ruined my ruin as I was forced to give up and return to the saloon as no other options were possible to continue. If I had missed a step in this fight, please disregard this message as it was my first playthrough and encounter with this boss, but I had no other place to check or no hints to see anything else either as I had roamed the entire map for 10 minutes trying to find the solution.
