I fired up this save game after a few months and I noticed this with a few systems:

I thought it was a bug with the only I have that changes things, Endless Moons. I didn't remember these systems like this before. When I checked them out closer, the one icon says its a blackout system. I am playing the Vaulters and have the Penumbra DLC, but I can't find any info on Blackouts, like how they happen or what not.

Blackouts can happen if another empire manages to hack you successfully. You cannot control the system for a few turns. It's no bug.

I think the bug is the way the icons are showing on the system view for me, showing all those icons. I think it might be an issues with Endless Moons.

Ah, yes, I've seen it before. I can't remember if it was reported on G2G or Reddit, but it is definetely caused by Endless Moons.
