I upgraded one of my heroes' ships with a bunch of Titanum modules. Afterward, I didn't have an option to upgrade it to the new design like a normal ship. Does that mean hero ships are upgraded automatically once you save the design?

Indeed, and you will see that you cannot upgrade if you can't afford the resource cost (when exiting the ship design screen).
