I have a friend who's helping me to mod ES2, and we play together a lot of the time. I don't think she's actually purchased any of the Endless titles (because we mostly play together). Are the avatars mostly all tied to buying the games, to a certain number of gtg points, or will some unlock for her over time?

Most of the avatars are indeed tied to game badges, each badge unlocking G2G pts, avatars, titles, DLCs, ... There is still some avatars not tied to a game badge, like the Survivor, which is unlocked overtime.
